An EMS Voluntary Event Notification Tool from EMSForward

EMS EVENT LogoEVENT is a tool designed for employees and professionals working in the Emergency Medical Services field/industry to submit voluntary, anonymous reports to a non-regulatory body regarding events that may or may not have resulted in injury to a patient or provider.

  • Patient safety events, whether or not the patient was harmed
  • Patient safety near misses, events that did not reach the patient but could have
  • Unsafe condition (opportunity for improvement)
  • Assault/violence directed at provider

General complaints about an organization or EMS agency should be submitted thru appropriate channels with your state, provincial, or territorial regulator. 


The goal is to improve the safety, quality and consistent delivery of care among Emergency Medical Services (EMS) personnel working in ground, air and water ambulance services, and operating in all delivery models. The collected data will be used to identify gaps in the delivery of care and design better policies, procedures, and training programs.

This tool was initially created with sponsor support from the Savvik Foundation, the National EMS Management Association (NEMSMA), the Paramedic Chiefs of Canada (PCC), the National Association of Emergency Medical Technicians (NAEMT) and the National Association of State EMS Officials (NASEMSO).

Any individual who encounters or recognizes a situation in which an EMS safety event occurred, or could have occurred, is strongly encouraged to submit a report by completing the EVENT online form. The confidentiality and anonymity of this reporting tool is designed to encourage EMS practitioners to report EMS safety events without fear of repercussion.
